The BoA Health and Wellbeing Team are here to support the health and wellbeing of patients 18+ throughout Bradford-on-Avon and Melksham to help individuals make a positive and lasting change to their health with a personalised approach. We aim to encourage patients every step of the way, giving them the tools and techniques to last a lifetime. The team is made up of Social Prescribers, Health Coaches and Care Co-Ordinators who work together alongside the surgery to provide the best possible care tailored to patients needs.
How to Access the Service
Any member of staff at the surgery will be able to refer you to the team.
You will then be contacted by us to arrange an initial consultation. This first appointment may be by phone, face to face at our wellbeing hub or as a visit.

Measure your Pressure
High blood pressure can lead to heart attacks, strokes and other illnesses. Once you ‘Know Your Numbers!’ you can get support to bring your blood pressure under control and prevent these diseases.
Around a third of people in the UK have high blood pressure, but most don’t know it. It doesn’t have any symptoms so, the only way to find out is to have a blood pressure check.
You can measure your blood pressure in our waiting rooms, at the two pharmacies in Bradford on Avon,

If you require PPE – for example to care for someone who lives in a different household, or if you or the person you care for has been exposed to Covid – it is now possible to order it through a new portal.
